6th - 8th Grades

There are two steps to the enrollment process for new students:
 
1. Complete the online enrollment application: Enrollment Application
 
2. Schedule an appointment here: MV Middle
 
Appointments are scheduled on Central Time Zone. Please ensure your device is on the correct time zone. You must schedule one appointment per child and allow a minimum of 30 minutes per appointment. If your online application is not complete at the time of your appointment, your appointment will be cancelled. 
 
The child's biological parent or legal guardian must be the one to come to the appointment to complete the registration and submit the following documents:
  • Child's birth certificate
  • Child's social security card
  • Child's immunization records
  • ID of parent / guardian (picture ID must be current, expired IDs will not be accepted)
  • Active military ID (if applicable)
  • Proof of residence stating physical address and / or service address (light bill, water bill, property tax statement, new buyer's contract or new lease agreement - may not be more than two months old). The bill must be in parent's name, current, show usage and state service address matching student enrollment application. Expired, past due or disconnection notices will not be accepted.

  • 504 Records
  • Speech Education Papers
  • Legal Documents
