You will also need the following required documents :
Child's original birth certificate
Child's social security card
Child's up-to-date immunization records
Proof of Residence (stating physical address and / or service address) in the parent/guardian's name:
- A current month or the prior month's utility bill; full page (electricity, water, or gas bill); or
- A new lease, or rental contract (full document; signed within the last 30 days)
- A closing disclosure for a home purchase (full document; signed within the last 30 days. Pre-closing documents are not accepted.)
Parent/Guardian's Driver's License
High School students - prior year report card or transcript
